Family Home Health Network, is a provider of intermittent Home Health care, Palliative Care, Hospice Care and Custodial Private Care Services throughout the Chicagoland area.

We are seeking a dedicated Home Health RN with a strong commitment to quality patient care. To succeed in this role, you must possess excellent clinical nursing and customer service skills. You will be responsible for completing a detailed initial assessment of the patient, the family, and the home environment to determine specific home health care needs, monitor and regularly re-evaluate the patients nursing needs, and initiate the appropriate plan of care as well as making necessary revisions as patient status and needs as they change.

• Salary determined on experience and licensure

Qualifications:

• Must possess current Illinois RN licensure
• Possess a minimum of two (2) years experience (Home Health Preferred)
• Current CPR
• Valid IL driver’s license and auto insurance policy
• WellSky (Kinnser) experience a plus
• Must possess excellent clinical nursing and customer service skills
