Job Description:

• Evaluate clients & develop a treatment plan that will meet their needs.
• Choose activities that will help the clients to carry out their daily works independently.
• Monitor clients as they carry out the task given to them.
• Observe & assess clients progress and prepare detailed reports accordingly.
• Maintain safe and clean working environment.
• Maintain documentation of client care services by auditing records.
• Maintain a cooperative relationship among healthcare teams & contributes to team effort.

Qualifications/Requirements:

• Graduate of an accredited Physical Therapy Program.
• Current New York State PT License.
• Experience in Assisted Living settings preferred.
• Proven exceptional customer service and patient care experience.
• Self-motivated with the willingness to exceed patient expectations.
• Ability to work in a fast paced environment while maintaining a positive attitude.

Job Type: Full-time

Pay: $82,000.00 - $113,345.87 per year
