We are seeking a Social Worker for the Care Manager position in our Tifton office location. Social Worker Care Managers arrange and manage the care of the elderly and disabled clients in community settings. Our Care Managers provide No Hands-on Care.

This position requires:

· Bachelor’s Degree in Sociology, Psychology, or similar field

· Someone who is dedicated to quality work.

· Someone who is goal oriented and is an organized self-starter.

· Computer experience

· Experience in home health or with Medicaid waiver programs or similar work is required.
