Basic Job Duties:
Supports the Waiver department by recommending and communicating patient needs and identifying possible community resources. Assure that the plan of care is followed as a member of your case management team.
Requirements:
LLBSW, LBSW, LLMSW, or an LMSW with at least one year experience as a Social Worker in the State of Michigan required. Positive, interpersonal skills. A thorough understanding of confidentiality and HIPAA principles.
Preferred Qualifications:
Experience with DHS (Medicaid) and working with the elderly population preferred.
