Position Summary: Under the supervision of the Cottage Director, the Shift Supervisor acts as support for the Cottage Manager, Program Coordinator, Recreation, Behavior and Program Specialist. Shift Supervisor ensures Cottage adaptive equipment is available for the residents use and that staff are trained on the adaptive equipment. Works with the other managers on duty to ensure all groups including 1:1’s are covered and rearranges DSP’s if necessary, to meet the requirements of the Cottage; including assigning DSP’s to attend appointments, outings, etc. while maintaining ratios in the building. Ensures all residents needs are met through supervision, care and assistance by providing direct assistance in Activities of Daily Living. Oversees the completion off all documentation. Works as a DSP when coverage is needed. Covers for Program Support Tech in the afternoons and when necessary.

Job Duties:

· Provides initial and on-going staff training of the PARC Cottage residents use of Adaptive equipment, meal cards and procedures, wheelchairs, Hoyer lift, gait belts, repositioning, and walkers as needed, in accordance with the PARC Cottage philosophy, policy, procedures, and ICF/DD guidelines while at the Cottage and ADT.

· Identify and promptly report any behavioral or physical change in residents to QIDP, Nurse, Social Services, Behavioral staff, Program Coordinator or Manager. Identify and initiate resolutions. Ensure safety and health needs of all residents and efficient flow of two-way communication.

· Ensures residents have the adaptive equipment needed to be successful and that it is in good condition/working order.

· Ensures completion of all daily paperwork, programmatic data, daily reports, incident reports, behavior reports, medical paperwork, progress notes, etc.

· Attends and participates in standing committee meetings as requested.

· Able to communicate clearly and professionally.

· Provides care and assistance to residents throughout the shift, to include bathing, dressing, toileting, repositioning, and dining. Ensures the individuals needs are met.

· Coordinates and assists at mealtime ensuring adaptive equipment is present and eating protocols are being followed.

· Identifies broken and potentially hazardous situations and completes Maintenance Requests or notifies the Cottage Manager immediately.

· Communicate resident and building emergencies to the appropriate Supervisor.

· Rearranges staff and ensures the correct ratios are maintained especially if there are call outs, breaks, appointments, trainings, outings, etc. Takes the group until someone can relieve them.

· Attends scheduled trainings and in-services and demonstrates the learned competencies.

· Completes daily plans on weekends and holidays. Assigns DSP’s to outings, makes necessary revisions and ensures ratios are maintained. Places corrected plans in Social Service Coordinator’s mailbox.

· Completes building checks frequently, especially after sending a group out for an outing or an appointment to ensure the ratio is being maintained.

· Assigns staff to transport residents, needed equipment or pick up residents from ADT as needed or at the end of the day. Attends doctor appointments and/or assigns staff to attend doctor’s appointments.

· Maintain and repair wheelchairs, and adaptive equipment, bed rails, bed rail pads, bed alarms, air mattresses, shower chairs through ordering and scheduling repairs.

· Complete proficiency checks during meals and day program. Maintain the ADT adaptive equipment bin and counts of Adaptive equipment and Aprons

· Ensures the staff have all the household supplies, hygiene supplies and clothing/shoes needed for the residents.

· Communicates in a professional manner with staff, parents/guardians, therapist, and visitors.

· Implements behavior protocols/programs according to policy.

· Ensures staff are following proper policy and procedures for safety, infection control and housekeeping.

· Able to work with a diverse population of people.

· Rotating on-call

· Demonstrates knowledge of facility evacuation and emergency procedures.

· Ensures PARC and Cottage Policies and Procedures are followed.

  • Proficiency in basic computer programs.
  • Ability to obtain and maintain PARC approved Professional Crisis Management, CPR, and First Aid certifications.
  • Ability to obtain and maintain PARC drivers’ licenses

Education and Experience

  • High school diploma, or equivalent GED

· Must have 1-year of experience in working with individuals with developmental disabilities
