Heritage Home Health Services continues to grow in the Indianapolis area! We are seeking full-time RN's for the Northeast (Fishers, Noblesville, Carmel, Muncie) and South (Beech Grove, Southport, Greenwood) and the surrounding area who can help improve the well being of our patients and their families and has the heart and skills to work with home bound patients. Area of coverage will be determined by location of candidate. **Qualified candidates may be eligible for a sign on bonus up to $10,000!**

• Work with the physician for medication needs and changes to the plan of care.
• Manage other disciplines such LPNs, aides, therapy staff to give the best care needed for each patient.
• Ability to complete OASIS, recert, d/c, ROCs documentations within an EMR system
• Help decrease the need for re-hospitalization by being actively involved in the patient's plan of care to assess any changes that may be needed.
• When appropriate teach and counsel family members about care and processes for disease/injuries.
• Attend orientation, meetings and CEUs,

Requirements

Computer skills; must be able to work within EMR system.

RNs must have current Indiana RN license in good standing.

Reliable auto and current auto insurance.

Current CPR.

Can work well independently

RNs Solid understanding of Medicare/Medicaid guidelines as it applies to Home Health

Pay: $65,000.00 - $75,000.00 per year
