Position Overview

The Pregnancy Quality Coordinator will support the mission and vision of Five Rivers Health Center by coordinating pregnancy-related quality initiatives. Provides administrative support to the Centering Pregnancy and Parenting programs. Responsibilities include organizational and reporting duties, as well as collaborating closely with the providers, instructors, and other staff to assist in planning, implementation, coordination and evaluation of Centering sessions. Coordinates care for patients to work towards improvements in early access to prenatal care, postpartum care, low birth weight, maternal/infant mortality, and other pregnancy-related metrics. Collaborates with quality and women’s health teams to identify areas of need and initiates quality projects to work towards improvement. Supports maintenance of the Comprehensive Maternal Care (CMC) program activity and quality requirements.

General Responsibilities

• Coordinate care for pregnant women to ensure early access to care and postpartum care
• Manages rosters of pregnant patients using Azara, Excel, Epic, and/or payer portals. Uses reporting to inform coordination of care
• Initiates, leads, and/or collaborates to develop quality improvement projects related to pregnancy care

Qualifications

• High School Diploma required
• Bachelor’s Degree preferred
• Excellent communication, organizational and PC skills required
• Epic experience preferred
• Minimum one year experience with direct patient communication required
• Minimum one year experience working directly in a prenatal care setting preferred
• Strong customer service skills and a thorough understanding and working knowledge of patient/client confidentiality issues and HIPAA compliance/regulations will be essential

Pay: $16.50 - $18.50 per hour
