General Responsibility:
Responsible for coordination and monitoring of medical record system
Core Functions
1. Sort, scan, and file all documentation pertaining to clinical record and address problems/concerns regarding medical record.
2. Prepare discharged records for scanning in established format.
3. Prepare external request for review of medical record for Palliative only.
4. Manage FAIRHOPE death certificates.
5. Scan and file all palliative paperwork, to include admissions and discharges.
6. Assist with Performance Improvement process through Medical Records Audit.
7. Serve as backup to Medical Records Technician 1 as needed.
8. Prepare medical records for storage and/or shredding.
9. Fax IDG team summaries weekly.
10. Verify Physician Licensure per agency policy and update monthly.

Competency Requirements
Education/Experience
- High School graduate preferred, with office experience. Certification in Medical Records preferred.
- Knowledge and experience in the health care field.
- Knowledge of office machines.
- Must be able to read, write English and carry out instructions.
- Ability to manage time efficiently.
- Must have and maintain a valid driver’s license, good driving record and auto accident insurance at a level acceptable to Fairhope and/or Fairhope’s insurer.
- Equivalent education/experience may substitute for minimum qualifications except when there are legal requirements, such as a license/certification/registration.
Physical Requirements
The Physical Requirement Rating reflects the estimated overall strength requirement of the job. It represents the strength requirements, which are important for average, successful work performance. The strength rating is expressed by one of five terms: (Mark applicable rating.)
S* Sedentary Work*: Exerting up to 10 pounds of force occasionally (Occasionally: activity or condition exists up to 1/3 to 2/3 of the time) to lift, carry, push, pull, or otherwise move objects, including the human body. Sedentary work involves sitting most of the time but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally, and all other sedentary criteria are met.
L* Light Work*: Exerting up to 20 pounds of force occasionally, and up to 10 pounds of force frequently, and/or a negligible amount of force constantly (Constantly: activity or condition exists 2/3 or more of the time) to move objects. Though the weight lifted may be only a negligible amount, a job should be rated Light Work (1) when it requires walking or standing to a significant degree; or (2) when it requires sitting most of the time but entails pushing and/or pulling of arm or leg controls; and/or (3) when the job requires working at a production rate pace entailing the constant pushing and/or pulling of materials even though the weight of those materials is negligible.
M* Medium Work: * Exerting 20 to 50 pounds of force occasionally and/or 10 to 25 pounds or force frequently and/or greater than negligible up to 10 pounds or force constantly to move objects. Physical requirements are more than those for Light Work.
H* Heavy Work: * Exerting 50 to 100 pounds of force occasionally and/or 25 to 50 pounds of force frequently and/or 10 to 20 pounds of force constantly to move objects. Physical requirements are more than those for Medium Work.
V* Very Heavy Work*: Exerting more than 100 pounds of force occasionally and/or more than 50 pounds of force frequently and/or more than 20 pounds of force constantly to move objects. Physical requirements are more than those for Heavy Work.

Evaluation
Initial evaluation at 90 days; thereafter annually on date of hire by immediate supervisor.

Pay: $16.89 - $24.31 per hour
