Home Health RN, per diem, full time

We are looking for qualified Home Health RNs to serve out patients in the Greater Sacramento as we expand our home infusion and home health team. The ideal Registered Nurse candidate will have a minimum of 1-2 years of experience with direct patient contact, visit nursing, dressing and wound care, experience with peripheral vascular access and infusion delivery systems.

Home Health Registered Nurse RN
Responsibilities of the Home Health Registered Nurse RN

• Coordination with patient the scheduling of visits
• Perform infusions in the home setting. PICC, port and PIV access.
• Short and long infusions (IVIG, antibiotics, specialty drugs, etc.)
• Some cases will involve visit nursing with wound care in conjunction with physical therapy
• Assure proper maintenance of clinical records in compliance with all applicable laws
• Develop a Plan of Care with the patient and family
• Communication with office, physician and pharmacy as needed

Requirements of the Home Health Registered Nurse RN

• NO LESS THAN 1 year of experience with direct patient contact as a licensed RN, per our accreditation requirements
• Minimum of 1 year of experience with peripheral vascular access and infusion delivery devices
• Visit nursing experience preferred
• Dressing and wound care a plus
• Exceptional nursing assessment skills and excellent interpersonal skills
• Case management skills
• Current and unrestricted RN license in CA
• CPR and TB skin test
• Valid Driver's License, reliable car with auto insurance and ability to travel on a daily basis
• Must be willing to submit to a background check and drug screen
• Ability to clock in and out on a Smartphone

Pay: $26.00 - $52.00 per hour
