Rocky Mountain Care has an immediate opening for a full-time Intake/Admissions Nurse

Responsibilities and Duties

• Receive information from referral sources and coordinate care for patients
• Verify and enter physician orders
• Identify level of care needed
• Receive and give verbal orders accurately
• Review clinical documentation to determine Skilled Nursing placement
• Exceptional interpersonal and customer service skills
• Ability to communicate clearly with pharmacist
• Work closely with referring hospitals, physicians, skilled nursing facilities, patients, and families
• Manage after-hours and weekend referrals (rotating on call)

Qualifications:

• Current license as a licensed LPN/RN from the Utah State Board of Nursing
• 1-year Skilled nursing facility experience
• Good verbal and written communication skills
• Basic computer knowledge
• Ability to effectively problem solve
• Provide excellent customer service
• Knowledge of Medicare/Medicaid home care and hospice criteria preferred
• Knowledge of Medicare, Medicaid and 10A criteria

Schedule

• Monday – Friday (rotating shifts: 8:30-5:00pm / 10:00-6pm)
• One schedule weekend every 8-12 weeks, and rotating monthly on-call

Job Type: Full-time

Pay: $27.00 - $30.00 per hour
